Liability Waiver Agreement & Consent of Participation:
I acknowledge and agree that dancing as in any other situation involving physical activity, involve risk of bodily injury including but not limited to those of partial and/or total disability, paralysis and death. There may also be other risks not known to me or that reasonably foreseeable at this time. I acknowledge that these risks and dangers may be caused by the negligence of the students or the negligence of others. I accept and assume such risks and responsibility for the loss and/or damages following such injury, disability, paralysis, or death, however caused or alleged to be caused in whole or in part by the negligence of the student, EB Bollywood Dance School, its officers, directors agents and employees, other student instructors, or owners and lessees of the premises used to conduct the activities. I agree that I – the parent/guardian is totally responsible for supervising the above named student and his/her activities, and his/her total safety, outside the teaching room or after the scheduled class time.
I also exempt, release and indemnify EB Bollywood Dance School, it's owner, agents, volunteers, assistants, employees, guest artists, faculty members and/or students any and all liability claims, demands or causes of action whatsoever from any damages, loss, injury or death to me, my child or property which may arise out of connection with participation in any class or activities conducted by EB Bollywood Dance School.
I agree that the consent of participation covers each and every activity and event organized/sponsored by the EB Bollywood Dance School. I also confirm that the above named student(s) has no problems health or otherwise that might compromise his/her safe involvement or that of others. I also have read and agreed to all the rules stated on the "Policies" section on EBB school website www.DanceBollywood.net.

Authorization of Medical Care:
Should my child/above named student(s) become ill or injured while at EB Bollywood Dance School class location OR its events, and I, the parent/guardian cannot be reached, I hereby empower the staff of EB Bollywood Dance School, to authorize medical care and treatment for the above named student(s).

Photo/Video Rights Release Agreement:
Permission is granted to EB Bollywood Dance School to record/use photographs/videos of my student(s) for publicity purposes. I authorize and agree that EB Bollywood Dance School may take and use photographs, videos or likeness of myself or my child as needed for its record keeping, advertising, social media and/or public relation projects and that I have no rights to the same and will not be compensated for the same.

General Disclaimer:
Furthermore, I/we agree to read and follow the class and faculty policies and to take full responsibility for my child’s/our behaviour in addition to any damage I/we may cause to the facilities utilized by EB Bollywood Dance School. I acknowledge that the school policies are published and regularly updated under the "Policies" menu on the school website www.DanceBollywood.net.

To set right expectations for parents of little beginners:
Unlike fun/game activities, starting phase of most learning activities, might encounter a “starting reluctance” from many little beginners, especially if they are a bit shy to mingle OR lazy to put effort OR have attention problems OR don’t like instructions or rules! (After all, they are the ones who’ll be benefitted the most by a team learning like EBB)
Learning group dancing, takes lot of focus, observation and discipline in a group. Most 3-6 year olds need to tune/train themselves to get the learning technique. For many little ones, they might find the first few sessions of learning, harder than what they expected of "dancing"! Parents along with the teacher have to keep them motivated and stay patient & determined for them to get over with this starting phase. It might take a few classes to a few months, for them to gradually master how to focus, watch, listen and pay 100% attention! Gradually they’ll start turning confident and show faster progress. That's the point they'll really start enjoying the fun of dancing & working with a group! Once they master the techniques of learning an activity like group dancing, they'll be ready to learn anything under the sun! We'll keep communicating to parents and get your beginner reach that point, as early as possible!
